Enroll During Your Initial Enrollment Period
It is important for individuals who are new to Medicare Part D to enroll in a Medicare prescription drug plan as soon as they become eligible. This is called the Initial Enrollment Period. Enrolling during the Initial Enrollment Period allows an individual to avoid paying any late-enrollment penalties.
Note:Certain people, such as those who receive Medicaid assistance, may be allowed to enroll at other times and may not be subject to a penalty. For more information, contact the plan sponsor.
How to Enroll
It is best to contact the company sponsoring the plan directly to get specific information about enrolling. Generally, this can be done through one or more of the following methods:
- Phone
- Fax
- Regular mail
After the company receives a completed application from the enrollee, it will obtain approval from Medicare and send the new member everything he or she needs to begin coverage.
What you need to fill out a Medicare Part D application
- Basic personal information including name, date of birth, and street address
- Medicare claim number (located on your red, white, and blue Medicare card)
- Social Security number
- Preferred premium payment method (invoice, credit card, automatic deductions from Social Security checks or bank account, etc.)
- E-mail address
- Emergency contact
